Description

An exciting opportunity has arisen for a Care Coordinator to join our Team and support the Health Promotion Team and clinicians.

Your main duties will include, but are not limited to, the following:

  • Organise and coordinate weekly rotas of care staff.
  • Liaise with clients and their family on a regular basis.
  • Ensure that the staff rotas are organised and sufficiently well planned, taking into account travel times and routes.
  • Work with recruitment and Human Resources Teams to ensure sufficient current and future staffing levels.
  • Plan and allocate care assignments to appropriate staff, with the upmost regard for the particular care needs of the client.
  • Liaise, as necessary, with allied health professionals with the goal of delivering holistic, client-centred packages of care.
  • Deal with queries from clients and families in a calm and professional manner at all times.
  • Carry out on-call coordinating duties outside normal office hours.
  • Regularly update client notes and care requirements as and when needed, as well as ensuring the client notes are prepared and in the home for all visits.
  • Effective communication with care staff, ensuring they are meeting all of the client’s care requirements and are updating the office regularly.
  • You may occasionally also be required to take on tasks outside the normal scope of this job description dependant on business requirements.
