We are currently looking to recruit a band 5 Mental Health Nurse into the Older Peoples Service Community Mental Health Team. We are looking for a committed and motivated practitioner who wishes to consolidate their practice in community services and develop their skills, experience and knowledge.

Main duty of the job

• The successful candidate will be responsible for the comprehensive assessment, planning, implementation, and evaluation of individual care needs for this client group. The candidate will have a good knowledge and understanding of how to assess and escalate risk within these settings. There is a requirement to keep up to date records using electronic care systems.
• As a band 5 practitioner you will provide professional input into the Multi-Disciplinary team which comprises of professionals from multidisciplinary backgrounds, working collaboratively to provide specialist focused mental health interventions. As an autonomous practitioner you will be required to provide care co-ordination within the framework of the CPA process. Service User involvement is at the centre of our approach, and you will embrace this in your practice. You will be able to demonstrate a range of therapeutic interventions and be committed to the concept of recovery whilst working alongside the wider community provision in Leeds.
• You will need to be able to work independently out in the community. It is essential that you have a full UK driving licence and use of a car. The successful applicant will be offered a comprehensive continuing professional development package which will include access to appraisal, clinical and managerial supervision.
